It starts almost like a domestic drama, following a successful, controlling man as his personal life quietly falls apart, before tipping into scenes of graphic, hands-on cruelty once his mind snaps. The pacing lingers on unease and obsession with cleanliness and decay before delivering its nastiest moments, so the dread builds steadily rather than jumping out at you. Its scares come from real, human violence and body horror tied to dental tools, not ghosts or monsters, which makes the discomfort feel a little too close to home for some viewers. There's a streak of dark, campy humor running through it, but the dental torture scenes are graphic enough to leave a real sting after watching.
